The Eastern Terminus of WV 46 is at WV 28 in Fort
Ashby.
(H.B. Elkins)
The terminus is a signalized intersection. A left turn from WV 28
North puts you on WV 46 West.

The Western Terminus of the eastern segment of WV 46 is at the Potomac
River in Piedmont. WV 46 crosses into Maryland and reappears again
in West Virginia a few miles to the west. This is looking into the
Potomac River town of Piedmont.
(Michael Muiznieks)
Looking into Piedmont's sister town of Westernport. WV 46 becomes
MD 36 once crossing the Potomac here.
Western Segment:
(Michael Muiznieks)
WV 46 returns to the Mountain State after re-crossing the Potomac River
a few miles to the west of Piedmont near Luke, MD. MD 135 is located
at the T intersection in the background of the photo. Interesting
contrast with the MD 135 and WV 46 shields. The MD 135 sign is mounted
on a wooden sign post, and the WV 46 'END' combination is on a WV standard
steel post.

This 12 mile segment of WV 46 consists of a gravel roadway. Recently,
the road was paved, although it is a very thin coating and no lines painted.
I believe WV 46 was the last WV State Route to be unpaved.
(Seth Dunn)
The road then returns to better pavement and ends at WV
42 in Elk Garden.
