The Northern Terminus of WV 18 is at WV 2 in Sistersville.  WV 18 used to end in town at the landing of the Sistersville-Fly (OH) Ferry; however, a two lane bypass rerouted WV 18 to the south of town.

The Southern Terminus of WV 18 is at WV 47 in Troy.
