The Western Terminus of WV 15 is at WV 4 near Flatwoods.
US 19 used to follow this section of WV 4 until the completion of I-79
in the area. Despite the older WV 15 sign, WV 15 was extended in
the 1980s from WV 20 in Webster Springs to WV 4 in Flatwoods by way of
Guardian.

The Eastern Terminus of WV 15 is at another US/WV multiplex, 219 and 55,
at Valley Head. The terminus is located at the southern end of a
very enjoyable stretch of US 219/WV 55 from Elkins
to this point. US 219/WV 55 begins a very mountainous ride south
of the WV 15 terminus.
