Western Terminus

OH 646's western endpoint is along the northern shore of Tappan Lake in central Harrison County, at a "T" intersection with US 250. There is no END signage for OH 646 at this location. US 250 is signed as an Ohio Byway in this part of Harrison County, given its scenic nature.

Eastern Terminus

At its eastern end, OH 646 intersects OH 43 in Wintersville. The JCT OH 646 and destination signs shown here are on northbound OH 43, just north of an interchange with the US 22 freeway.
